Boy Scout Summer Camp Forms

  • 2008 Camp Leaders' Guide - The guide for all Scout Leaders attending Fire Mountain this season (Large File).
  • 2008 Boy Scout Camp Registration Form - Registration form for Mt. Baker Council Boy Scout Summer Camps.
  • Photo Permission Slip - Release to be signed by each Scout's Parent/Guardian, which allows the Mt. Baker Council to use photos of Scouts in camp for promotional purposes.
  • Troop Roster Form OR Troop Roster Form (Microsoft Excel) - Please fill out this form completely before arriving at camp.
  • Pre-Camp Swim Check Form - If you would like to take your unit's Swim Check before you arrive at camp, have a BSA or Red Cross Lifeguard administer this test and fill out this form. Be sure that they attach a copy of their certification. This is an excellent way to make your check-in day easier.
  • Class 1-2 Medical Form - Each Scout will need the front and back of this form filled out by a doctor within the last 36 months, and the parental permission on the bottom of the front page signed and dated within the last 12 months.
  • Class 3 Medical Form - Leaders who are age 40 or over need to use this form (class 1 portion for those in camp under 72 hours, the whole form for those in camp 72 hours or longer).

  • National Tour Permit
  • Provisional Camping Form OR Provisional Camping Form(Microsoft Word File) - For a Scout who will be attending a Mt. Baker Council Camp without his unit. See Camp Leaders Guide for more information.
  • Make sure you bring your troop's accident insurance policy number with you to camp!
  • Be sure to bring your enthusiasim and excitement for a great week at Fire Mountain!
